Clinical Care Coordinator

We are seeking a Clinical Care Manager to work in our Boston area office. The Clinical Care Manager is the front line of intake and support for our customers and business partners. These contacts may be inquiring about employee assistance program (EAP) services and behavioral health referrals through other established programs. The Clinical Care Manager will manage intake calls from individuals and corporate clients, assess needs, record demographic and insurance information, and determine appropriate next steps to engage with a therapist or behavioral health specialist.

In this role, the key areas of immediate emphasis will include:

· Providing telephonic and electronic assessment and referral and/or short-term problem resolution and referral for members or their family members.

· Performing initial triage of calls and performing crisis stabilization, where warranted.

· Frequently making decisions to adjust his/her schedule to manage crisis calls and determine and provide appropriate critical clinical intervention.

· Following defined customer workflows to verify eligibility in programs.

· Following up on cases in accordance with customer requirements.

· Responding to after hours and weekend calls and referrals for assistance when needed.

· Staff Client cases with Licensed Clinicians and behavioral health providers throughout the US, including providing case management support throughout the process.

· Preparing and providing reports to internal stakeholders and customers as required.

Education

· Bachelors’ Degree required. Master’s degree in mental health field preferred.

· Licensed in a US state as Mental Health Counselor or Social Worker.

Experience

· Minimum 5 years of mental health or clinical experience required, preferably in a health care environment.

Necessary Skills

· Excellent written and verbal communication skills.

· Excellent interpersonal communication.

· Strong organizational and time management abilities.

· Solid experience with technology, including but not limited to Microsoft Outlook, Excel, and Word and other Office 365 technologies.

· May require working outside normal hours for crisis or referral situations.

Licensed Clinician
